Declaration Form: Covid-19 Affected Countries and Regions

Club to introduce Health and Travel Declaration Form from tomorrow, Wednesday 18 March


Dear Members,

As the outbreak of Covid-19 is now officially a global pandemic, the Club is extremely concerned about the health and safety of its members, their families, their guests, visitors and our staff members.

It is with regret, therefore, that with effect from tomorrow, Wednesday 18 March, anyone who has visited Covid-19 affected countries and regions in the past 14 days will be asked to refrain from using the Club’s facilities, including the courses.

Similarly, please avoid using the Club if you have been in close contact with a traveller from the affected countries and regions in the past 14 days. This would include a family member in your household, for example.

To be clear, the countries and regions of concern are those which require incoming travellers to undergo compulsory quarantine from as specified by the Centre for Health Protection (click here) and / or those which have a “Red” Outbound Travel Alert issued by the Security Bureau (click here).

These countries and regions are subject to change, and we ask all members to refer to the CHP and SB websites going forward.

Please also be aware that the Club will be closed to all reciprocal member play for the time being.

We will continue to monitor the situation extremely closely and may implement further measures at short notice.

Health and Travel Declaration Form

Everyone entering the Club – including members, member guests, visitors and staff – will be asked to sign a Health and Travel Declaration form declaring that they have not visited any of the countries and regions concerned.

Furthermore, they must declare that they have not been in contact with anyone with a confirmed case of Covid-19 in the past 14 days and are not experiencing any flu-like symptoms.

The form, which must be completed every time you visit the Club, will be available at both Deep Water Bay and Fanling. You can also download the form here, in order to print it out and complete before your arrival to save time.

Completing the form will contribute to helping us maintain the safety of everyone who uses the Club in addition to our staff members, and provide peace of mind.

It is of utmost importance that all information and declarations provided are true and correct. And at this time we would like to remind all members that they are responsible for the actions of their guest(s).

Please also note that the accommodation at Fanling must not under any circumstances be used by members as a place to self-quarantine.

We thank you for your understanding and co-operation with the measures that the Club has put into effect to reduce and minimise health risks to all. It is of the greatest importance.
